Refer a patient to BlueCare

We can provide care and support to patients of all ages, for government subsidised or privately funded services. We will just need your patient's MAC ID if they are being funded under CHSP.

Would you like to discuss a BlueCare patient? Our new Health Professional Patient Hotline uses your patients phone number to identify which local care team your call needs to be directed to.

Calling your patients local BlueCare team

  • Have your patients mobile or landline number available
  • Call 1800 030 289, selection option 2
  • Input your patients phone number
  • Your call will directed to the relevant care team
